Leadership Inquiry

Would you as a parent want to hear more about the roles that help organize our swim team? For example Chair Positions, Board Positions, Committees. This is without any obligation to committe to anything. *

Photo Release Agreement

I hereby grant permission to CWST to use photographs and/or video of my swimmer during CWST sponsored events ONLY in publications, news releases, online advertisement, and in recruting communications related to CWST swim club. *

Parent Volunteer Agreement

Parent participation throughout the season is required to follow SHRSL rules & regulations. CWST has a variety of tasks and duties based on your interest and time of availability and can accept volunteer work from either parent or even a swimmer or sibling in some cases (must be 16 yrs. of age). All families have volunteer responsibilities totaling 5 work shifts for the season.

I UNDERSTAND MY INVOLVEMENT IS MANDATORY. I further understand that there are extreme cases where families are unable to meet the mandatory work shift minimum, and there is a $300 volunteer buyout fee that is available for me to pay with registration to exempt my family from volunteer responsibilities. I understand if by the last event of the season, my required 5 working shifts are NOT fulfilled. The credit card used for my swimmer(s) registration will automatically be charged. I hereby grant CWST permission to charge the credit card used for my swimmer(s) registration a total of $300, if I or any member (16 yrs. or older) of my family DO NOT fulfill the mandatory 5 working shifts.

● I will demonstrate good sportsmanship at all practices, meets, and other swim events.

● I will make sure to set a good example of behavior and sportsmanship for my younger teammates.

Code of Conduct Agreement- Parent

The purpose of a code of conduct for parents is to establish consistent expectations for behavior by parents. As a parent/guardian, I understand the important growth and developmental support that my child’s participation fosters. I also understand that it is essential to provide the coaching staff with respect and the authority to coach the team. I agree with the following statements:

  •  I will set the right example for our children by demonstrating sportsmanship and showing respect and common courtesy at all times to the team members, coaches, visiting teams, officials, parents, and all pools.
  •  I will get involved by volunteering, observing practices, cheering at meets, and talking with my child and their coach about their progress.
  •  I will refrain from coaching my child from the stands during practices or meets.
  •  I understand that criticizing, name-calling, use of abusive language or gestures directed toward coaches, officials, volunteers, and/or any participating swimmers will not be tolerated.
  • I will respect the integrity of the officials.
  • I will direct my concerns to first the coaching staff; then, if not satisfied, to the appropriate CWST Board Member.
  • I understand that my swimmer(s) will be evaluated on their swimming abilities and that the coaches have the final decision as to whether they meet the requirements for participation.

I understand the above expectations and that my failure to adhere to them may result in disciplinary action. I understand that any misconduct by the above swimmer(s) may result in disciplinary action up to and including removal from the team in accordance with the Swimmer Code of Conduct Agreement. The Novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

The Southwest Houston Recreational Swim League and teams associated therewith (SHRSL) have done their best to follow the State of Texas guidelines and put in place preventive measures to reduce the spread of COVID-19; however, SHRSL cannot guarantee that you will not become infected with COVID-19. You understand that the risk of becoming exposed to or infected by COVID-19 may result from the actions, omissions, or negligence of SHRSLvolunteers.

You voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to yourself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability or expense, of any kind, that you may experience or incur in connection with your attendance at SHRSL events (Claims). You hereby release, covenant not to sue, discharge, and hold harmless SHRSL, its agents, representatives, board and volunteers, from the Claims. You understand and agree that this release includes any Claims based on the actions, omissions, or negligence of SHRSL, its agents, representatives, board and volunteers, whether a COVID-19 infection occurs before, during, or after participation in any SHRSL event.
